Each student is responsible for ensuring that all pre-requisite courses are completed prior to the start of classes. This form must be submitted prior to the deadline for adding courses in the term or session. Submission of this form does not constitute a waiver of any course pre-requisite. Requests must be initiated prior to the last date to add courses for the term. If your request is approved, you will be automatically registered for the course(s) listed.
